GPs should continue to issue an FP10 prescription for cylinder oxygen until the patient has been contacted by the new supplier to arrange transfer to the new service arrangements. Until the supplier contacts the patient, the patient should be advised to request a repeat prescription where oxygen supplies need to be renewed.
BOC will inform the GP when we have contacted the patient and will confirm the date of transfer and that the required service is being delivered to the patient. As part of arrangements for transfer to the new service, BOC will request the patient to contact the pharmacy to arrange return of pharmacy cylinders and headsets.

Any registered healthcare professional can place an order for Home Oxygen. Typically this will include general practitioners, members of the specialist home oxygen assessment service and ward clinical staff.

• There are two forms that you will need:

1. The Home Oxygen Order Form (HOOF)
2. The Home Oxygen Consent Form (HOCF)

• The HOCF should be completed at the same time as a HOOF and is used to obtain patient consent for release of personally identifiable information to the oxygen supply company.
• Both forms should be completed in full as omissions are likely to result either in delay in supply or a call from Vitalair for clarification.
• Once completed the HOOF should be faxed to Vitalair on 0800 169 9989
• You will need to file the original with the patients notes and send copies to:

1. the PCT or Common Services Agency
2. the Trust Clinical Oxygen Lead
3. the Patient
4. the Patient's GP

• The completed HOCF should be copies (front page only) and filed in the patients' notes. A copy should be given to the patient.
• When Vitalair has received the order form we will advise you and the patient of the planned date of installation and deliver the oxygen equipment directly to your patient's home. This will be approximately 3 days from receipt of the HOOF.
• You will not need to write repeat order forms each time your patient needs a new cylinder. Vitalair will continue to deliver your patient's oxygen requirements until we are notifed otherwise.
